Leaf 217 (second count) misnumbered 215.
The appendix, collating [par.]² A-D⁶ [sec.]² and beginning with a note to the reader, may be bound in various places in the two volumes. Leaf 12 (first series) is misnumbered 11. Pagination above shows the appendix at the end of vol. 2.
Another state (STC 841) has "Printed by the assignement of the Companie of stationers" in the imprint.
The whole entered in stationer's register to the English Stock on March 5, 1620.
