Preceding the ephemerides, each with caption title, are: "A treatise of the name, definition, distribution, and vse of an ephemeris", "The second part, entreating of mathematicall physicke, or a briefe method of physicke, by iudiciall astrologie", and "The third part of this treatise, handleth the iudgements of the astrologians upon certaine questions appropriated to the 12 houses".
"Although the text of the dedication is the same setting in all copies, the dedicatees vary: Earl of Craven (British Library copy), Sir T. Middleton, kt. and bt. (Folger Shakespeare Library copy), Sir H. Berkeley, kt. and bt. (American Antiquarian Society copy), S. Marow (Harvard University Library copy)."--STC.
